Serves as the Social Work Program Manager for Homeless Programs, providing leadership and oversight of program operations, clinical services, staff supervision, resource management, and program evaluation. The incumbent functions as the subject matter expert for homeless services, ensuring quality care delivery, regulatory compliance, strategic planning, and effective utilization of resources across multiple VA sites.

- →Duties include but are not limited to: Direct and manage the administrative, operational, and clinical functions of the Homeless Programs.
- →Coordinate interdisciplinary staffing, work assignments, treatment services, admissions criteria, outreach efforts, and program activities.
- →Oversee the delivery of homeless services across multiple locations, including the medical center and Community Based Outpatient Clinics (CBOCs).
- →Plan, organize, implement, and evaluate program operations to ensure effective and efficient service delivery to Veterans experiencing homelessness.
- →Manage program budgets, fund control points, resources, and operational priorities.
- →Develop, implement, monitor, and evaluate policies, procedures, clinical guidelines, protocols, and performance measures.
- →Collect, analyze, and utilize program data to assess performance, improve quality of care, enhance patient outcomes, and support program evaluation.
- →Develop and maintain systems to track referrals, visits, inpatient utilization, patient demographics, equipment utilization, and patient satisfaction.
- →Conduct program planning, analysis, assessment, research, monitoring, and continuous quality improvement activities.
- →Provide administrative, technical, and clinical leadership to staff assigned to the Homeless Programs.
- →Establish performance expectations, assign work, monitor productivity, conduct performance evaluations, and provide ongoing coaching and feedback.
- →Develop performance improvement plans, recommend personnel actions, address employee relations matters, and implement disciplinary actions when appropriate.
- →Review and approve leave requests and ensure adequate staffing coverage to support program operations.
Requirements
