About the Role
The incumbent serves as program coordinator at the GS-12 grade and is clinically and administratively responsible for the development, implementation, and management of the Veteran Directed Care Program (VDC) within the VA Eastern Colorado Healthcare System. The incumbent manages the daily operation of the program, develops policies and procedures for program operation, and prepares reports and statistics for facility, VISN, and national use.
What You'll Do
- →Administers information and analytical systems to evaluate and enhance the quality of service provided.
- →Develops and implements information systems to track service activities including visits, referrals, patient demographics, equipment utilization and patient satisfaction.
- →Performs a wide range of services and provides technical assistance to State, local, and private agencies, and universities.
- →Technical assistance provided is based on monitoring activities and program reviews, and the development and analysis of information.
- →Uses sound judgment in the interpretation of trends and patterns; the application of guidelines; and the recommendation of program enhancements.
- →Performs program analysis, development, review, research, design, and contact work as possible with various agencies (public, DOD, state, etc.).
- →Performs work related to the review, monitoring, analysis, planning, and assessment of Veteran Directed Care (VDC).
- →Responsible for key project activities related to the planning, development, and implementation of VDC.
- →A substantial portion of the workload directed by this position is carried out at one or more locations that are physically removed from the medical center.
- →Performs clinical duties that involve specialized treatment of complex physical or mental illness.
- →The incumbent incorporates complex multiple causation in differential diagnosis and treatment of veteran patients, including making psychosocial and psychiatric diagnoses within approved clinical privileges or scope of practice.
- →Provides individual, group, and family psychotherapy and advanced level case management intervention used in the treatment of veterans with polytrauma injuries, spinal cord injuries, traumatic brain injuries, visual impairment, post-traumatic stress disorder, addictions, and other serious disorders.
- →Develops and implement methods for measuring effectiveness of social work practice and services in specialty areas, utilizing outcome evaluations to improved treatment services.
- →The incumbent is able to demonstrate the ability to design changes based on empirical finding.
