About the Role
The social worker serves as a member of an interdisciplinary treatment team contributing to overall primary healthcare services to Veterans assigned to a primary care team. The social worker provides clinical counseling, psychosocial assessment, and case management services to eligible veterans and their family members to meet biological, psychological, social, and environmental needs. The social worker adheres to the principles of the Patient Aligned Care Team (PACT) Model.
What You'll Do
- →The assessment will establish facts about presenting problems and causes, as well as impact of problems on the Veteran's daily functioning and health.
- →Demonstrates the ability to interpret data collected in the assessment process to determine treatment needs, elevated risk factors for suicidality or gaps in essentials for basic living, navigate frequent or severe crises or significant lack in support systems.
- →Treatment Planning/Goal Setting-Develops treatment plans that address psychosocial needs identified by the Veteran and thorough assessment.
- →Collaborates with the Veteran and the support persons of the Veteran's choice in the development of achievable goals to address problems.
- →Referral to Service Providers-Collaborates with both internal and external service providers in assessing the Veteran's needs for programs, services and entitlements.
- →Resource Development-Develops a resource directory or file that is germane to the area of assignment.
- →The directory or file will comprehensively list both VA and community resources and social service programs for which Veterans may be eligible and through which the Veteran may benefit.
- →Advocacy-Understands the complexities of a large healthcare system and will act as an advocate when required or serves the best interest of the Veteran and family.
- →Crisis Intervention-Makes rapid assessments and developing crisis management plans.
- →Accesses to immediate medical attention for the Veteran from primary care staff should medical or psychiatric emergency occur.
- →Supportive Counseling-Provides Veterans and their families with supportive counseling, short-term solution focused or brief problem-solving therapeutic modalities.
- →Documentation and Administrative Functions-Documents interventions and interactions with Veterans using appropriate templates, note titles, CPT codes and documentation procedures for timeliness and content.
- →Work Schedule: Monday through Friday, 7:30 am to 4:00 pm Recruitment Incentive (Sign-on Bonus): Not Authorized Permanent Change of Station (Relocation Assistance): Not Authorized Pay: Competitive salary and regular salary increases.
- →Paid Time Off: 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year) Selected applicants may qualify for credit toward annual leave accrual, based on prior [work experience] or military service experience.
