About the Role
The main duties of this position involve delivering social work services to PACT, GM, and Specialty clinics at the VA Ann Arbor Healthcare System. The person in this role is responsible for providing outpatient social work services to Veterans who primarily receive their medical care in the Patient Aligned Care Team (PACT) clinic.
What You'll Do
- →Total Rewards of a Allied Health Professional Major duties of this position include, but are not limited to: Responsible for providing Outpatient Social Work services to Veterans who are receiving medical care predominately in the Patient Aligned Care Team (PACT) clinics.
- →Functions as the subject matter expert with regards to VHA/VBN Federal, State and local government programs, benefits and services available to our Veteran population.
- →Demonstrate a high level of skill in assessment, intervention and treatment plan development when addressing the comprehensive psychosocial needs of the Veterans He/she is assigned.
- →Evaluate the patient to identify any emotional, social, physical, family-related, financial, or vocational challenges that might be contributing to distress, affecting their health condition, or creating obstacles to receiving the recommended care or treatment.
- →Conduct a comprehensive assessment to identify the underlying causes of the presenting issues, as well as any interpersonal and environmental factors that may be contributing to the situation.
- →Offer a considerate approach to help the patient and their family understand the factors contributing to the identified issues.
- →Discuss available short-term and long-term solutions or resources, and utilize a strengths-based empowerment model to develop a patient-centered intervention plan aimed at reducing stressors, enhancing health, and achieving health-related goals.
- →Actively engage as a member of the interdisciplinary treatment team, collaborating closely with Veterans, their families and caregivers, as well as Non-VA providers.
- →Offer consultation to the treatment team and staff regarding the psychosocial needs of Veterans, including the effects of identified psychosocial barriers on healthcare planning and treatment compliance.
- →Provide information about available interventions, programs, and services that can help address and resolve these barriers.
- →Identify appropriate VHA, VBA, and community resources, and facilitate their utilization and connection by the Veteran and family, to help them achieve optimal functioning.
- →Paid Time Off: 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year) Selected applicants may qualify for credit toward annual leave accrual, based on prior [work experience] or military service experience.
- →Parental Leave: After 12 months of employment, up to 12 weeks of paid parental leave in connection with the birth, adoption, or foster care placement of a child.
- →Child Care Subsidy: After 60 days of employment, full time employees with a total family income below $144,000 may be eligible for a childcare subsidy up to 25% of total eligible childcare costs for eligible children up to the monthly maximum of $416.66.
