About the Role
The Patient Aligned Care Team (PACT) is part of a comprehensive team which delivers primary care to Veterans in either a longitudinal or episodic fashion, which has as its focus on the following: prevention; health promotion, psychoeducation, care coordination, direct clinical services, and chronic disease case management.
What You'll Do
- →VA Careers - Social Work: https://youtube.com/embed/enRhz_ua_UU Total Rewards of a Allied Health Professional Duties may include but are not limited to: Primary responsibility to assist PACT Veterans to navigate care, facilitating access to care, and follow-up with the PACT team and ensure appropriate care is being provided.
- →Incumbent will establish and maintain professional interactions and relationships with stakeholders both within the VA and the Community.
- →The incumbent utilizes and promotes group, individual, and phone education to complete Advance Care Planning (ACP) and/or Advance Directives in the PACT clinic.
- →The incumbent educates Veterans on the use of VA Virtual/Video Connect (VVC) clinics effectively, The incumbent has the ability to independently evaluate his/her own practice through participation in professional peer review case conferences, research studies, or other organized means.
- →The incumbent will serve on committees, work groups, and task forces at the facility and/or Veterans integrated Services Network (VISN) level and/or in the community.
- →Work Schedule: Monday - Friday, 8:00am - 4:30pm Recruitment Incentive (Sign-on Bonus): Not Authorized Permanent Change of Station (Relocation Assistance): Not Authorized Pay: Competitive salary and regular salary increases.
- →Paid Time Off: 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year)Selected applicants may qualify for credit toward annual leave accrual, based on prior [work experience] or military service experience.
- →Parental Leave: After 12 months of employment, up to 12 weeks of paid parental leave in connection with the birth, adoption, or foster care placement of a child.

Requirements
