About the Role

The Patient Aligned Care Team (PACT) is a comprehensive, multidisciplinary team which delivers patient-centered primary care. The social worker embedded within the PACT setting functions as a care coordinator to ensure Veteran's health care needs are met, in collaboration with the Veteran and their personal support person(s).

What You'll Do

  • Total Rewards of a Allied Health Professional Social workers are uniquely prepared for this role by virtue of their education, training, and experience which prepares them to understand the person-in-environment and the influences that family and community have in impacting the health care needs of Veterans.
  • The incumbent is responsible for a variety of duties, including but not limited to, the following: Must have skill and expertise to establish and maintain effective therapeutic relationships with Veterans in the Primary Care Clinic and/or their personal support system.
  • The incumbent is able to independently work with Veterans and their families who are experiencing a wide range of complicated medical, psychiatric, emotional, behavioral, financial, legal and psychosocial problems.
  • Effective social work interventions and delivery of care in the PACT setting often occurs over a series of several visits.
  • Have knowledge of the PACT team approach and the role of each health and allied health care team members as well as an understanding of the role of the social worker in the PACT model.
  • Independently complete thorough psychosocial assessments to determine the psychosocial functioning and needs of Veterans and/or their personal support team.
  • The incumbent is able to assess functional acuity of the Veteran and provide appropriate interventions.
  • The incumbent must be able to utilize this assessment in facilitating the Veteran's maximum use of treatment for attainment of the highest level of independence that is possible and practicable.
  • Actively participate as a member of the interdisciplinary treatment team and actively participate through collaboration with Veterans and personal support system as well as interdisciplinary treatment team members in the development and implementation of treatment goals and interventions.
  • Must possess a working knowledge and experience in use of medical and mental health diagnoses, disabilities, and treatment procedures, including acute, chronic and traumatic illnesses, substance abuse disorders, common medications and their effects/side effects, and medical terminology.
  • Must serve as a liaison between Veterans and/or their personal support system and VA and community resources to ensure thorough delivery of services.
  • Must possess and demonstrate ability to communicate effectively, both orally and in writing with people from varied backgrounds.
  • Work Schedule: Monday - Friday, 7:30am - 4:00pm or 8:00am - 4:30pm Recruitment Incentive (Sign-on Bonus): Not Authorized Permanent Change of Station (Relocation Assistance): Not Authorized Pay: Competitive salary and regular salary increases.
  • When setting pay, a higher step rate of the appropriate grade may be determined after consideration of higher or unique qualifications or special needs of the VA (Above Minimum Rate of the Grade).
