About the Role
About the Position: This position is located at 15th MDG JBHP-Hickam, Directorate of Medicine Medical Operations, Family Health, Hickam Air Force Base, Honolulu, HI. This is a Direct Hire Solicitation. Advance in Hire may be authorized. This is available for those candidates who are new to Federal service.
What You'll Do
- →Conduct patient assessments and delivers Primary Care Behavioral Health appropriate biopsychosocial evaluations using advanced knowledge of concepts, principles, and practices in clinical, health, and primary care psychology.
- →Conduct brief assessments and formulates evidence-based interventions appropriate to a primary care setting for a wide range of behavioral health conditions, chronic medical conditions, and health behaviors affecting medical conditions.
- →Assist with implementation and monitoring of treatment plans with Primary Care Managers (PCMs) and other primary care team members.
- →Provide ongoing consultation services for a subset of patients who would benefit from periodic, regular behavioral health monitoring and intervention for sustaining improvement and preventing relapse.
- →Work to increase overall penetration into the enrolled clinic population by targeting common conditions with behavioral components that are amenable to Behavioral Health Clinic (BHC) intervention.
