About the Role
The Inpatient Medicine social worker provides clinical social work, case management services to Veterans admitted to inpatient medicine, surgery, and ICU. Services are timely, comprehensive, and effectively coordinated with services provided by other health professionals. The Inpatient Medicine Social Worker works as a part of an interdisciplinary team to provide psychosocial health behavior interventions and referrals to assist in successful discharge planning.
What You'll Do
- →Total Rewards of a Allied Health Professional The Inpatient Medicine social worker delivers timely, comprehensive clinical social work and case management services for Veterans in inpatient medicine, surgery, and the ICU.
- →Working as part of an interdisciplinary team, they coordinate care with other health professionals and provide psychosocial interventions and referrals to support effective discharge planning.
- →The incumbent is responsible for a variety of duties, including but not limited to, the following: Completing timely admission screening/health behavior assessments.
- →Providing education on advance directive planning and assisting with completion of advanced directives.
- →Formulating a treatment plan and goal setting for discharge planning.
- →Supporting the highest level of functioning of the Veteran including discharge to independent living, when possible, or long-term care facility when a Veteran requires skilled nursing facility.
- →Ability to establish and maintain effective working relationships with clients, staff, and representatives of community agencies.
- →Ability to communicate effectively, both orally and in writing, with people from varied backgrounds.
- →Develops working relationships and agreements with other organizations having responsibilities for the same patient population.
- →Maintains a current and productive network of referral resources.
- →Make provision for referral services from other agencies, community resources (i.e., transition or halfway houses, shelters, etc.), and other Governmental agencies to resolve problems.
- →Knowledge of community resources, how to make appropriate referrals to community and other governmental agencies for services, and ability to coordinate services.
- →Implementing a variety of different treatment modalities when working with individuals, families and groups to achieve treatment goals.
- →Completing timely documentation and approved methods for management of records (paper, electronic, electronic systems).
Requirements

- ✓Selective Service Registration is required for males born after 12/31/1959.
- ✓Must be proficient in written and spoken English.
- ✓Subject to background/security investigation.
