About the Role
This position is a Social Worker with duty station located at the Sisseton Service Unit, Sisseton, South Dakota. This position reports to the Behavioral Health Supervisor or Designee. A REAL ID will be required beginning May 7, 2025, in accordance with 6 C.F.R. 37.5 (2021).
What You'll Do
- →Assess and evaluate patients using a patient centered medical home approach using therapeutic techniques to diagnose and treat mental health diagnosis.
- →Develops interventions based on above need assessment and exercises mature professional judgment in consultation with the supervisory social worker in providing therapeutic modalities that are culturally appropriate.
- →Participates and when appropriate takes a leadership role in assessing community human service needs.
- →Works with community members and human resources -providers to develop approaches to addressing the needs of patients.
- →Assists in monitoring and reviewing services for quality and appropriateness of care.
- →Assists in developing as needed appropriate services to address patient and family psychosocial needs.
- →Prepares reports as requested on quality improvement activities.

Requirements
