Social Worker - BHIP Care Coordinator

About the Role
This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.
What You'll Do
- →Provides consultation/education to Veterans and families on VA resources (including self management tools), community resources, Advance Directives, VA benefits, and options for community care as appropriate.
- →Provides care coordination (including care management or case management) to Veterans and families, as needed, throughout the continuum of care including within and across services/departments/sections to facilitate continuity of care for Veterans.
- →Provides wellness/prevention education and may facilitate patient/family educational/bridging supportive sessions as appropriate.
- →Consults, collaborates, and coordinates with other staff who provide care to Veteran.
- →Administrative Duties (10-20%): Takes lead in ensuring accuracy of PCMM, maintaining accuracy of data integrity dashboard, and monitoring and addressing BHIP team performance measures.
- →Participates in program planning and development, advising the supervisor of shifting trends with care coordination needs and caseloads and identifying the need for making recommendations on program procedures and/or policy changes based upon work experiences.
- →Monitors the efficiency, effectiveness, timeliness, quality of services offered within the program area.
- →Reviews relevant data, assesses the data, develops, and implement ideas for improvement.
- →May provide consult triage and consult management duties to facilitate the timely provision of mental health services.
- →Work Schedule: Monday-Friday, 8:00am-4:30pm Recruitment Incentive (Sign-on Bonus): Not Authorized Permanent Change of Station (Relocation Assistance): Not Authorized Pay: Competitive salary and regular salary increases.
- →When setting pay, a higher step rate of the appropriate grade may be determined after consideration of higher or unique qualifications or special needs of the VA (Above Minimum Rate of the Grade).
- →Paid Time Off: 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year).
- →Selected applicants may qualify for credit toward annual leave accrual, based on prior [work experience] or military service experience.
- →Parental Leave: After 12 months of employment, up to 12 weeks of paid parental leave in connection with the birth, adoption, or foster care placement of a child.
