About the Role

This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.

What You'll Do

  • Total Rewards of a Allied Health Professional The GS-11 BHIP Care Coordinator Social Worker is a full-time position assigned to the Behavioral Health Outpatient Clinic located at the Bath VHA.
  • The incumbent must use a high level of skill in assessing and managing the needs of complicated psychosocial problems of Veterans.
  • The responsibilities of this role include case management, advocacy, and coordinating care within the VA or community service providers.
  • This is done in collaboration with other mental health treatment teams providers, and other interdisciplinary treatment teams.
  • Additionally, the provider will conduct mental health triage and treatment matching assessments for Veterans referred to outpatient mental health services, as well as intake sessions for high-risk individuals.
  • Crises intervention is also a part of this position.
  • Collaborate with BHIP leadership/team members to develop/enhance educational materials to promote BHIP care coordination services.
