About the Role

The Senior Social Worker assigned to the Polytrauma System of Care is responsible for direct inpatient and outpatient care as a member of a interdisciplinary treatment team(s). Veterans and active-duty service members treated at this regional site come from a wide geographic area, presenting a necessity for the social worker to build and foster networks for referral and care coordination.

What You'll Do

  • The incumbent must function with a high degree of independence and knowledge of VHA regulations to ensure appropriate referrals into the polytrauma system of system of care and community.
  • The incumbent demonstrates knowledge of DoD, as well as other government agencies, in order to interface with them on behalf of the patient.
  • The incumbent will develop and implement methods for measuring effectiveness of social work practice and services in specialty rehabilitation areas including the PRC, PNS, PTRP, IETP settings along with other related programs.
  • They will utilize outcome evaluations to improve treatment services.
  • Incumbent will be able to demonstrate the ability to design changes based on empirical findings.
  • The incumbent will write end of year reports analyzing outcomes and process improvement efforts.
  • They will collaborate with medical director on agenda for monthly inpatient TBI team meetings and annual team retreat.
  • The incumbent will be able to offer consultation to colleagues, interns, and students on the psychosocial treatment of patients treated in rehab specialty areas, rendering professional opinions based on experience and expertise.
  • They may provide clinical supervision to other social work staff who are working to achieve full performance licensure.
  • The incumbent will orchestrate patient movement, including admission and discharge planning, with veterans, active duty military, families, DOD counterparts, and community stakeholders within and outside the region.
  • The incumbent will demonstrate the ability to teach and mentor staff and students in the special area of practice and will be able to provide supervision for licensure or specialty certifications.
  • The incumbent will complete other duties as assigned.
  • Work Schedule: 08:00 to 16:30; Monday through Friday Telework: Ad-Hoc only.
