About the Role

The VAMHCS Primary Care Senior Social Worker provides social work services at the Glen Burnie Community Based Outpatient Clinics (CBOC). Services include primary health care, medicine, surgery, mental health and substance abuse treatment, home health care and geriatrics/long term care, respite, hospice, palliative care, and emergency care.

What You'll Do

  • Major Duties Include: Conducts psychosocial assessments.
  • Requires the development of treatment plans in collaboration with the veteran/family and with the interdisciplinary treatment team.
  • Possesses advanced knowledge and ability to assess and treat the unique bio-psychosocial needs of returning combat veterans.
  • Independently performs case work and outreach, identifying and screening patients for psychosocial needs.
  • Coordinates community-based services, including information and referral for additional services from other VA programs, other government programs and community agency programs.
  • Establishes and maintains effective therapeutic relationships with veterans and their families.
  • Provides consultation services to other staff regarding the psychosocial needs of veterans and the impact of psychosocial problems on health care and adherence to treatment plan.
  • Facilitate referrals based upon needs and eligibility.
  • Provides the customer with consistent information according to established policies and procedures.
  • Knowledge and experience in use of medical and mental health diagnoses, disabilities and treatment procedures.
  • Knowledge and skill in the use of word-processing, data management and other computer systems, especially those programs in use by VHA.
  • Consistently communicates and treats customers (veterans, their representatives, visitors, and all VA staff) in a courteous, tactful, and respectful manner.
  • Handles conflict and problems in dealing with the consumer constructively and appropriately.
  • Paid Time Off: 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year) Selected applicants may qualify for credit toward annual leave accrual, based on prior [work experience] or military service experience.
