About the Role

This position is for the Veteran Directed Care Program and the Homemaker/Home Health Aide Coordinator. It focuses on providing expanded care options for veterans who opt to receive extended in-home setting and community-based services, as opposed to institutional care. The H/HHA Coordinator is responsible for maximizing psychosocial quality of life for veterans living at home within the community, with the goal of reducing hospitalizations and postponing the need for nursing home placement.

  • The incumbent performs the following duties as assigned, but is not limited to: Coordinates the work responsibilities of clinical and support staff assigned to the program.
  • Oversees the screening, approval, modification, and recertification of services for Veterans in the program and ensures that Veterans approved for services meet VA eligibility requirements.
  • Establishes and maintains an effective working relationship with staff at the Area Agencies on Aging and Disability (AAAD) which provide VDC services to Veterans in the Memphis VAMC catchment area as well as any state agencies which provide oversight to the AAADs.
  • Assists with establishing and maintaining the annual VDC budget.
  • Conducts outreach to Memphis VAMC staff to market the program.
  • The H/HHA coordinator overseas program development, recruitment, and development of the community services related to the homemaker home health aide services, home care needs, recruitment and placement of veteran residents, case management, development of policies and care processes, and coordination with HBPC interdisciplinary treatment teams.
  • Has extensive knowledge of Medical Center policies and procedures, VHA and VBA regulations.
  • Performs other duties as assigned by the Community Program Supervisor.
  • Paid Time Off: 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year) Selected applicants may qualify for credit toward annual leave accrual, based on prior [work experience] or military service experience.
  • Parental Leave: After 12 months of employment, up to 12 weeks of paid parental leave in connection with the birth, adoption, or foster care placement of a child.
  • Child Care Subsidy: After 60 days of employment, full time employees with a total family income below $144,000 may be eligible for a childcare subsidy up to 25% of total eligible childcare costs for eligible children up to the monthly maximum of $416.66.
