About the Role

The Residential Senior Social Worker is responsible for individual and group therapy and case management for Veteran patients on one of three VAMHCS residential programs. The Perry Point residential treatment programs include a homeless Domiciliary (including CWT/TR), a Psychosocial Rehabilitation Residential Treatment Program (PRRTP), and a Substance Abuse Rehabilitation Residential Treatment Program (SARRTP).The Residential Social Worker will be crossed trained across all residential programs.

  • Provides services in one of three VAMHCS residential programs.
  • The Perry Point residential treatment programs include a homeless Domiciliary (including the Compensated Work Therapy/Transitional Residence program), a Psychosocial Rehabilitation Residential Treatment Program (PRRTP), and a Substance Abuse Rehabilitation Residential Treatment Program (SARRTP).
  • Assists with local, State and Federal benefits and assists with applications as necessary.
  • Has experience and familiarity in working with previously unhoused individuals and those with numerous psychosocial stressors.
  • Provides other concrete social work tasks to include referrals for housing, transportation, and food as needed.
  • Schedules discharge appointments and coordinates transportation.
  • Incumbent will network both internally and externally to the facility and coordinate community-based services as may be necessary; this may include information and referral for additional services from other VA programs, other government programs and community agency programs.
  • Completes suicide risk screening, suicide risk evaluations, and suicide risk safety plans.
  • Documents all interventions in a timely fashion as dictated by standard operating procedures and policy.
  • Serves as a functioning member of the Interdisciplinary Team, offering social work insights and interventions to the team that may be comprised of, but not limited to, prescribers, nurses, psychologists, technicians, vocational staff, other social workers and program support.
  • Meets the needs of customers while supporting VA missions.
  • Consistently communicates and treats customers (veterans, their representatives, visitors, and all VA staff) in a courteous, tactful, and respectful manner.
  • Incumbent provides the customer with consistent information according to established policies and procedures.

Requirements
