About the Role
Primary purpose of this position is: The incumbent serves as the Senior Social Worker for the Polytrauma and Amputee Clinics. Organization/Department: VA North Texas Health Care System (VANTHCS) Social Work Service
What You'll Do
- →Total Rewards of a Allied Health Professional Duties include but not limited to: Screening/Assessment/Diagnosis: The Polytrauma Senior Social Worker provides services and independently evaluates the situation of Veterans and family members/significant others and arrives at a reasoned conclusion.
- →Treatment/Care Planning Works with patients and families who are experiencing a variety of psychiatric, medical and social problems.
- →Independently provides crisis management and develops psychosocial treatment plans in coordination with interdisciplinary team members, including goals for psychosocial clinical treatment.
- →Participates on an interdisciplinary team and recommends a course of treatment.
- →Counseling/Crisis Intervention The Senior Social Worker provides support and interventions regarding the psychosocial impact of coping with illness, stress related to the response or an acceptance of diagnosis, treatments recommended, need for additional VA services or community-based services.
- →Case Management / Resources and Referrals Develops a resource file of VA and community social service programs.
- →Finds a suitable means of treatment to help Veterans and/or significant others cope with end of life issues and adjustment to death and dying.
- →Poly-trauma/TBI Assisted Living Program Provides case management and site visits to Veterans who are placed in VA sponsored TBI rehabilitation programs in the community.
- →Advocacy Acts as an advocate when it serves the best interest of the Veteran/family.
- →When appropriate and feasible, conducts education and encourages the Veteran/family to advocate on own behalf, thus fostering a sense of independence and empowerment.
- →Patient Education/Health Promotion and Prevention Provides education related to the Veteran's VA entitlements, eligibility, services and programs, as well as community resources for medical and mental health needs.
- →Professional Evaluation/Education/Training/Supervision/Mentoring Manages evaluation of the professional services provided.
- →Devises methods of obtaining data and evaluates data to determine the effectiveness of treatment.
- →Identifies consistent qualities of successful treatment and of failed treatment.
Requirements

- ✓Selective Service Registration is required for males born after 12/31/1959.
- ✓Must be proficient in written and spoken English.
- ✓Subject to background/security investigation.
