This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.
What You'll Do
- →The Senior Social Worker - Mental Health Treatment Coordinator serves as a senior clinician on the assigned Behavioral Health Integration Program (BHIP) team and helps to manage consults, patient flow, admissions, care assignments, and the disposition of requests for Community Care.
- →This position practices in the North Florida/South Georgia Veterans Health System (NFSG VHS), will be located on site at Jacksonville Naval Air Station.
- →The incumbent participates as a member of a the multidisciplinary treatment team, however, the main role of this Senior Social Worker - MHTC is to manage the flow of new and established patients as well as Veterans requesting community care.
- →This could include assisting with Veterans calling multiple outpatient clinics for same day access.
- →The incumbent provides comprehensive assessments as well as individual, and group treatment (psycho-educational and psychotherapeutic) of a broad spectrum of mental health disorders.
- →Duties for this position include but are not limited to: Independently manage complex Veterans within their program area.
- →This will be determined based on the Veteran's acuity, severity of condition, diagnosis or other programmatic guidelines as defined within the section to which they report.
- →The Senior Social Worker - MHTC will function as the subject matter expert in addition to functioning as a lead social worker for their assigned team.
- →Involves working with new and established Veterans with mental health disorders and organizing patient flow within in the VA and the community across the clinic(s) in the most effective way possible.
- →Receives referrals from a variety of sources to include triage, consults, walk-in visits, phone calls, and warm hand-offs from internal/external stakeholders.
- →The Senior Social Worker is required to utilize available data sources to proactively identify patients for screening/triage, assessment, treatment planning, case management and care coordination.
- →Provides crisis intervention services, seeking to address the cause as well as presenting complaint, coordinates family conferences and serves as a liaison to family members/caregivers.
- →The Senior Social Worker has working knowledge and experience in use of medical and mental health diagnoses, disabilities, and treatment procedures.
- →This includes acute, chronic, and traumatic illnesses/injuries, mental health conditions, and common medical terminology.
