About the Role
Responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care. Assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care. Administers medications and procedures per established policies and guidelines. Influences care outcomes by collaborating with members of the interdisciplinary team.
What You'll Do
- →ACOS for Rehabilitation/Physical Medicine and RehabilitationOutpatient Core elements of performance include knowledge and active participation in the unit/program level quality improvement process and initiatives as well as customer service programs.
- →Assures accurate collection and analysis of data related to the care of patients assigned through concurrent daily chart review and retro review as needed.
- →This includes creating and maintaining the specific database for reporting and quality improvement initiatives and organizing performance improvement activities.
- →Participates in the management and evaluation of the Interdisciplinary Pain Rehabilitation Program for individual and groups of patients in the outpatient setting within the Physical Medicine and Rehabilitation Service.
- →This nurse demonstrates decision-making capacity in the coordination of services for patients of the program.
- →Participates in patient and family teaching and provides leadership by working cooperatively with other members of the patient care team; maintains standards for professional nursing practice in the clinical setting.
- →Work Schedule: Mon to Fri 8:00-4:30.
- →May require weekend and Holiday work Telework: Available.

Requirements
- ✓Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
