What You'll Do

  • The Clinical Transfer Coordinator Registered Nurse is responsible and accountable for the coordination, facilitation, and oversight of patient transfers and referrals within the Veterans Health Administration (VHA) and between VHA and community partners.
  • The incumbent executes all elements of the nursing process-assessment, planning, implementation, and evaluation-while ensuring safe, timely, and cost-effective transitions of care across the continuum.
  • Major Duties and Responsibilities: Coordinates all interfacility transfers, to include VA-to-VA and VA-to-community transfers, ensuring safe, efficient, and patient-centered transitions of care.
  • Serves as the central point of contact for all incoming and outgoing patient transfer requests.
  • Facilitates communication between referring and accepting providers (physician-to-physician and nurse-to-nurse) to ensure continuity of care.
  • Coordinates patient placement utilizing the Bed Management System (BMS) and VISN patient placement tools.
  • Ensures completion and effective communication of all required clinical documentation (e.g., consults, labs, imaging, progress notes) to receiving facilities.
  • Maintains accurate and timely documentation in CPRS, transfer logs, and electronic tracking systems.
  • Reviews and synthesizes clinical data from multiple sources, including the electronic health record (EHR), Joint Legacy Viewer (JLV), external medical records, and verbal reports.
  • Collaborates with the Medical Officer of the Day (MOD), Nursing Officer of the Day (NOD), and interdisciplinary teams to determine the appropriate disposition of incoming transfers.
  • Arranges patient transport via ground or air Emergency Medical Services (EMS), ensuring the appropriate level of care (BLS, ALS, CCT, Code 3) is provided.
  • Communicates patient condition and care requirements to EMS providers.
  • Completes and maintains transportation logs and all other required documentation.
  • Preferred Experience: 5+ years of bedside clinical nursing experience in med/surg, ED and/or ICU.
