About the Role
This is OPEN CONTINUOUS ANNOUNCEMENT and will remain open until August 14, 2026. The initial cut-off date for referrals of eligible application will be June 14, 2026 with subsequent cut-off dates every month. Eligible applications received after that date will be accepted on an ongoing basis and qualified candidates will be considered as vacancies become available.
What You'll Do
- →Provide comprehensive primary care and preventative care to an established panel of primary care patients, under the Patient Aligned Care Team (PACT) model.
- →The type of care is served to the Veteran population with diseases consistent to those seen in the general population include but are not limited to the following: acute and chronic illness, women's health, gender specific, mental health, and preventive care, along with coordination of care delivery.
- →A major focus is on preventative medicine.
- →Function as a leader in the PACT approach in the delivery of care, which emphasizes a team-based approach to healthcare.
- →Participate in weekly team meetings and provide innovative management ideas for patient case reviews.
- →Duties include, but are not limited to, history taking, physical examination, ordering appropriate labs, diagnostic tests, and consultations needed.
- →Formulate multi-disciplinary treatment plans.
- →Perform medication reconciliation.
- →Document all medical information in an electronic medical record.
- →Other duties as assigned.

Requirements

- ✓Must pass pre-employment physical examination.
- ✓Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP).
- ✓You may be required to serve a probationary period.
