The Fayetteville VA Medical Center is seeking a Licensed Practical Nurse to support our Geriatrics and Extended Care Service Line. This position is ONLY open to current VHA Federal Employees. Current Federal Employees: Please submit your most Current, recent SF50. We will not accept an AWARD or LWOP SF50 for verification.
What You'll Do
- →Minimum one (2) years of LPN experience in home care, primary care, geriatrics, medical surgical nursing, or long-term care.
- →Experience working with interdisciplinary care teams.
- →Demonstrated ability to perform independent home visits, including patient education reinforcement, vital sign monitoring, and chronic disease support.
- →Experience with safe patient handling techniques and navigating environments with limited equipment.
- →Experience with reusable medical equipment handling, nursing bag technique, infection control, and specimen/medication transport.
- →Proficiency in electronic health record documentation (CPRS preferred).
- →Experience caring for Veterans or adults with chronic, complex conditions, cognitive impairment, or functional limitations.
- →Demonstrated ability to manage variable environments, prioritize tasks, and work independently in the field.
- →Strong clinical judgment in assessing changes in patient condition and escalating concerns appropriately.
- →Excellent communication skills, including concise documentation and effective teamwork with RNs, providers, and interdisciplinary HBPC staff.
- →Demonstrated organizational and time-management skills, especially when planning multiple home visits across rural or semi rural areas.
- →Ability to support chronic disease self-management, medication reinforcement, and patient/caregiver teaching in the home.
- →Proficiency in home environment safety awareness, including identifying hazards, implementing safe practices, and adhering to HBPC safety guidelines.
- →Demonstrated reliability, flexibility, and professional conduct when representing the facility in community settings.
