About the Role

The PACT LPN carries out the most complex practical nursing care assignments, which include performing nonstandard and specialized activities sufficient to solve a range of complex nursing problems, under the general review of a Registered Nurse in the outpatient clinic. The LPN uses a systematic problem-solving process in the care of multiple patients with predictable health care needs to provide individualized, goal-directed nursing care.

What You'll Do

  • The Cincinnati VA Medical Center (CVAMC) is a two-division campus located in Cincinnati, OH and Fort Thomas, KY serving 15 counties in Southwest OH, Northern Kentucky, and Southeast Indiana with 6 Community Based Outpatient Clinics located in Bellevue, KY; Florence, KY; Lawrenceburg, IN; Hamilton, OH; Clermont County, OH, and Georgetown, OH.
  • The CVAMC also supports two off main campus locations in the Cincinnati area via an Eye Center and Mental Health Outreach Community Division.
  • Completed work should need only a general review by a registered nurse (RN) or Provider for appropriateness and conformity with established policies and procedures.
  • Demonstrates the ability to observe, identify, and respond to the Veteran's needs for care, including medication, equipment-assisted care, and Veteran/family education.
  • In organizing and delivering care, the LPN recognizes and considers emotional, cultural, spiritual, socio-economic, and age-related factors.
  • Reinforces instruction provided by other health care professionals.
  • Instructs patients and family members on principles of health, self-care and protocols for treatments and /or medications.
  • Utilizes teaching materials to promote self-reliance and independence including methods to prevent complications and facilitate recovery.
  • Observes for physical and/or emotional changes in Veteran's condition from prescribed medications/treatments, promptly and accurately documenting noted changes, and reporting any deviations from normal to RN or Provider.
  • Uses sound judgment and demonstrates the ability to recognize the need for initiating emergency measures when indicated and assist in response procedures for cardiac/pulmonary arrest, anaphylaxis, fire, disruptive behavior, bomb threat, external disaster, missing patient, and hazardous spills.
  • Promptly seeks the assistance of the RN or Provider.
  • Completes set-up and assists with complex gynecological procedures within the Women Veteran's Health Center only.
  • Actively seeks out educational opportunities to enhance nursing knowledge and skills, sharing new knowledge gained with other staff to improve and advance nursing practice.
  • Serves as a preceptor in orienting, educating, and training less experienced or technically skilled staff related to support duties for these more complex, specialized tests/procedures.

  • Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP).
