About the Role
This is a Not to Exceed June 30th, 2027 Intermittent position. PA is a credentialed health care professional who provides patient-centered medical care to assigned patients as a member of a health care team and who is authorized to practice with defined levels of autonomy and to exercise independent medical decision making within their scope of practice. This vacancy will be filled with either a Nurse Practitioner or a Physician Assistant.
What You'll Do
- →Duties include but not limited to: The Home Based Primary Care (HBPC) Physician Assistant (PA) is a position located in the Geriatrics and Extended Care Service (GECS) and functions as a Primary Care Provider in the HBPC Program.
- →The Home Based Primary Care (HBPC) Physician Assistant (PA) is a licensed practitioner who has demonstrated a high level of proficiency in patient assessment including performing physical diagnosis, developing a comprehensive treatment plan, prescribing medications, evaluating laboratory data, and assessing patients' and families' psychosocial functioning.
- →The PA functions as the HBPC provider for his/her HBPC patients and coordinates their care among all members of the health care team, including other HBPC staff, outpatient primary and specialty physicians, inpatient services, ancillary hospital services, and community organizations.
- →The PA also promotes preventative health care and facilitates patient and staff education.
- →The PA will provide daily support for the activities of the HBPC Team.
- →The incumbent will function autonomously as guided by approved clinical protocols and privileges, practicing to the full extent of their education, training, and their scope of practice.
- →Given the interdisciplinary team structure of the clinic, collaboration with other interdisciplinary team members is expected.
- →Clinical duties: The core elements for a PA Scope of Practice may include, but are not limited to: I.
- →Obtaining medical histories and performing physical examinations.
- →Providing and coordinating medical care for assigned patients in any care setting, including establishing diagnoses, formulating and implementing care plans, and providing follow-up care.
- →Evaluating outpatients to determine the need for further health care.
- →Ordering diagnostic studies and other special tests, such as Magnetic Resonance Imaging (MRI), Computerized Tomography (CT) scans, etc.
- →Carrying out health promotion, disease prevention, and patient education activities.
- →Ordering or obtaining laboratory specimens.
Requirements

- ✓Effective May 7, 2025, driver's licenses or state-issued identification cards that are not REAL ID compliant cannot be utilized as an acceptable form of identification for employment.
- ✓Must pass pre-employment physical examination.
- ✓Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP).
- ✓As a condition of employment for accepting this position, you will be required to serve a 2-year probationary period during which we will evaluate your fitness and whether your continued employment advances the public interest.
