About the Role
About the Position: Serves as a Mental Health Care Facilitator providing care integration, telephonic triaging, and patient education in a primary care setting. Recruitment or relocation incentives MAY be authorized for highly qualified candidates. This is a Direct Hire Solicitation
What You'll Do
- →Conduct clinical triage and symptom monitoring through structured phone and in-clinic assessments using validated tools.
- →Support coordinated mental health care by tracking patient progress, communicating updates to providers, and ensuring continuity of care.
- →Provide patient education and promote self-management, including SMART goal-setting and treatment-plan adherence.
- →Manage referrals and care transitions to internal services, community resources, and TRICARE network providers.
- →Document assessments and maintain caseload data, preparing required reports for program staff.
- →Deliver staff and patient education and participate in outreach to increase appropriate referrals and caseload engagement.
- →Participate in quality improvement and peer review, including chart audits, trend identification, and corrective action reporting.
- →Collaborate with multidisciplinary teams such as Mental Health Consultants and Nurse Case Managers to coordinate comprehensive care.
