About the Role
The Nurse (Care Coordinator-Ortho) is responsible and accountable for all elements of the nursing process when providing or supervising Veteran care. Assesses, plans, implements, and evaluates age-specific care. Coordinates care focused on Veteran education, self-management, and satisfaction. Follows established policies, collaborates with the interdisciplinary team to influence outcomes, and demonstrates leadership in managing complex Veteran care.

- →The Nurse (Care Coordinator-Ortho) executes position responsibilities that demonstrate expert leadership, experience, and creative approaches in providing complex patient management.
- →Provides patient management, care coordination, and discharge/disposition planning for inpatient and outpatient settings.
- →Duties include but not limited to: Complex Patient Management & Care Coordination Provides expert leadership and advanced clinical judgment to manage complex patient conditions in both inpatient and outpatient settings.
- →Coordinates comprehensive care, including patient management, transitions, and discharge/disposition planning.
- →Assists Veterans, families, and caregivers in identifying and accessing appropriate healthcare options for acute, chronic, catastrophic, or life-threatening conditions.
- →Supports patients with complex needs involving combat stress, traumatic brain injury residuals, addictions, and community adjustment challenges.
- →Coordinates care across multiple providers, levels of care, and sites within VA and community programs to ensure continuity.
- →Addresses psychosocial, medical, and nursing needs through active participation with the interdisciplinary care team.
- →Expedites timely progression of care from diagnosis to treatment, ensuring all steps are organized and patient-centered.
- →Clinical Evaluation, Data Management & Education Analyzes and manages laboratory and radiology results, ensuring timely communication of findings to providers.
Requirements
- ✓Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
- ✓All applicants tentatively selected for VA employment in a testing designated position are subject to urinalysis to screen for illegal drug use prior to appointment.
- ✓Applicants who refuse to be tested will be denied employment with VA.
- ✓Selective Service Registration is required for males born after 12/31/1959.
- ✓Subject to background/security investigation.
- ✓Selected applicants will be required to complete an online onboarding process.

- ✓Effective May 7, 2025, driver's licenses or state-issued identification cards that are not REAL ID compliant cannot be utilized as an acceptable form of identification for employment.
- ✓Must pass pre-employment physical examination.
- ✓Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP).
