About the Role
About the Position: This position is located at Blanchfield Army Community Hospital at Fort Campbell, Tennessee. The Fort Campbell region offers a balanced mix of military, affordable living, and community support, making it an appealing location for both service members and civilian employees. This is a Direct Hire Solicitation. Relocation/Recruitment incentive MAY be authorized. Salary negotiation may be available for those candidates who are new to Federal service.
What You'll Do
- →Manage health metrics and outcomes of the primary care population.
- →Coordinate the implementation of clinical practice guidelines to support healthcare staff and patients.
- →Serve as a liaison between health care providers, members of the primary care team, and Nurse Case Managers to identify beneficiaries with short- and long-term health care needs.
- →Provide data support to stakeholders in support of population health and medical management activities.
- →Propose regulatory and administrative changes to resolve identified problems within the medical facility.
- →Conduct population health and medical management program evaluation and feedback.
