About the Role

The VA Northeast Ohio Healthcare System is recruiting for a Licensed Professional Mental Health Counselor (LPMHC). The LPMHC will function within the Behavioral Health Interdisciplinary Program (BHIP) team in Psychology Service.

What You'll Do

  • Behavioral Health Interdisciplinary Program (BHIP) team-based care provides Veteran centered, recovery-oriented, and evidence-based services.
  • The Licensed Professional Mental Health Counselor (LPMHC) serves as a BHIP Associate, providing expert care to Veterans served by the VA Medical Center's interdisciplinary outpatient mental health clinic utilizing evidence-based interventions for Veterans with a wide range of mental and behavioral health conditions.
  • Major duties include, but may not be limited to: Provides case management which includes determining and monitoring care coordination needs over time, and facilitating follow-up and outreach as needed to promote proactive, personalized care and care continuity.
  • Follows national guidance and local policy and SOPs for mental health services.
  • Participates in program planning and development, advising the supervisor of shifting trends in caseloads and identifying the need for making recommendations on program procedures and/or policy changes based upon work experiences.
  • Provides a broad range of psychotherapeutic interventions, including evidence based treatments when appropriate, to Veterans and families as needed throughout the continuum of care.
  • Conducts biopsychosocial assessments to assess high risk factors, including suicidal and homicidal ideation, acute needs for VHA or community-based services, and to determine case management and treatment needs of Veterans.
  • Provides crisis intervention, in collaboration with the multidisciplinary treatment team, for Veterans who are experiencing severe social or mental health crises.
  • Demonstrates detailed knowledge of VHA programs and community services in an effort to act as liaison between VA and community resources, to include education and bidirectional communication that promotes seamless care.
  • Documents interventions and activities in patient's electronic health record within established timeframes to ensure appropriate hand-off communication and documentation when transferring patient care.
  • Work Schedule: Monday through Friday, 8:00 a.m.- 4:30p.m.
