About the Role
This announcement is recruiting for Licensed Practical Nurse's (LPN) for the Beaver County Veterans Affairs Outpatient Clinic with an anticipated start date in late 2026 or early 2027.
What You'll Do
- →VA Careers - Licensed Practical Nurse: https://youtube.com/embed/Ae85IP1Oiz4 Total Rewards of a Allied Health Professional Duties include but are not limited to: Main responsibility is daily management of the Primary Care Provider's (PCP) scheduled appointments and clinic flow.
- →Other responsibilities includes assisting the PACT team or other interdisciplinary team members with panel management and care coordination.
- →Such tasks include test result notification, patient education, discharge follow-up, and chronic disease management.
- →Serves as an active participant in planning, implementation and evaluation of care.
- →Participates in daily huddles and team meetings with PACT team members.
- →Demonstrates, in interactions, the ability to communicate effectively and professionally with both internal and external customers.
- →Is knowledgeable of and properly initiates nurse protocols and Standard Operating Procedures (SOP) to help deliver holistic care.
- →Confers with LPN TL, RN Care Manager, PCP, or appropriate person to deliver care when needed.
- →Uses motivational interviewing and health coaching to guide patients in achieving goals of health promotion and disease prevention.
- →Participates in panel management: Completes pre-clinic phone prevention up to 24 hours before scheduled appointment.
- →Assists with chronic disease management and other care coordination needs.
- →Use of telehealth technology to render care.
- →Completes view alerts assigned within 48 hours.
- →Provide New Patient Orientation when you encounter new patients.
Requirements
