About the Role
Demonstrate proficiency in practice based on conscious and deliberate planning. Self-directed in goal setting for managing complex client situations. Assesses, plans, implements, and evaluates care based on e age specific components. Assumes responsibility for the corrdination of care focused on patient educations, self-management, and customer satisfaction throughout the continuum of care. Follows procedures per established policies and guidelines.
What You'll Do
- →Duties Include but not limited to: Administer medications and treatments according to policy.
- →Monitor patient responses and promptly report changes.
- →Provide competent, evidence based care to patients.
- →Coordinates care with multiple providers across all levels and sites of care.
- →Medical device equipment uploads to patient charts if applicable.
- →Analyzes and manages laboratory and radiology results and provides information to the providers.
- →Utilizes databases to provide population management to track patient care through all episodes of care.
- →Relocation/Recruitment Incentives: Incentive may be authorized for those highly qualified newly appointed applicants.

Requirements
- ✓Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
- ✓All applicants tentatively selected for VA employment in a testing designated position are subject to urinalysis to screen for illegal drug use prior to appointment.
- ✓Applicants who refuse to be tested will be denied employment with VA.
- ✓Selective Service Registration is required for males born after 12/31/1959.
- ✓Subject to background/security investigation.
- ✓Selected applicants will be required to complete an online onboarding process.

- ✓Must pass pre-employment physical examination.
- ✓Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP).
- ✓You may be required to serve a probationary period.
