About the Role
The Geriatric, Research, Education and Clinical Center (GRECC) Clinical Nurse Specialist (CNS) is self-directed and executes position responsibilities that demonstrate leadership, experience, and creative approaches to management of complex Veteran care. The CNS is an expert clinician who demonstrates the ability to work effectively with the geriatric Veteran population, families or significant others, and with professional and supportive personnel who provide Veteran Care.
What You'll Do
- →Major Duties: Provides advanced clinical care to a defined geriatric Veteran population, independently managing a panel/program using evidence-based treatment guidelines.
- →Collaborates with Veterans, families, and the interdisciplinary team to develop, implement, and evaluate individualized plans of care.
- →Provides leadership on ethical issues, applies the ANA Code of Ethics, and assists Veterans with completion of Advance Directives.
- →Manages program resources to ensure safe, effective, and efficient care, including ordering durable medical equipment, prosthetic devices, and supplies to maintain Veterans in the home.
- →Implements and updates an educational plan for self and others, maintains current knowledge of geriatric and nursing practice, and uses professional standards to evaluate program/service activities.
- →Actively advances nursing and geriatrics through education, mentoring, and guidance of nursing staff, fellows, residents, students, and other trainees, including developing seminars and conferences on geriatric issues.
- →Continuously evaluates and improves practice and programs using standards, clinical practice guidelines, and Joint Commission requirements, and helps develop and test new models of geriatric care aligned with the GRECC mission.
- →Uses group processes and committees to identify, analyze, and resolve care problems; participates on committees that impact the care of older Veterans and establishes peer relationships across extended, ambulatory, and acute care.
- →Coordinates referrals and consultations (e.g., HBPC, rehabilitation therapies, respite, home health, Medicare-certified home care and hospice) to support homebound Veterans and keep them safely at home.
- →Initiates and participates in research, evidence-based practice, and performance improvement projects, disseminating findings through presentations, journal clubs, and practice changes to enhance quality and timeliness of care.

Requirements
- ✓Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
- ✓All applicants tentatively selected for VA employment in a testing designated position are subject to urinalysis to screen for illegal drug use prior to appointment.

- ✓Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP).
