About the Role
The Primary Care Licensed Vocational Nurse (LVN) provides high-quality, evidence-based care for a culturally diverse Veteran population; emphasizing patient centered care to veteran's ages 18 to 75 and above in the outpatient care setting. They must be able to promote a safe environment for all patients including those with altered sensorium due to medication or an acute or chronic illness. The LVN will perform assignments at the ACC Delta Clinic.
What You'll Do
- →The Licensed Vocational Nurse performs assignments in the Primary Care Clinic (Delta Clinic) as a member of the Patient Aligned Care Team (PACT).
- →Under the direction of the Registered Nurse or Nurse Manager, she/he is responsible for providing direct and/or indirect patient care to an assigned group of patients.
- →This care takes into consideration all physical, psychosocial, spiritual and cultural needs of this specific Veteran population.
- →Preferred Experience: 1-2 year Patient aligned care team (PACT) experience Out-patient clinic experience Work Schedule: Monday -Friday , 7:30am-4:00pm.
- →Hours may change due to the needs of the facility.
- →Telework: Not Available.
