About the Role
The Prevention Care Coordinator Registered Nurse demonstrates leadership in delivering and improving holistic care through collaborative strategies with others. Evaluates practice in an ongoing process, based on best evidence. Provides high-quality, evidence-based care for a culturally diverse Veteran population; emphasizing patient centered care to veteran's ages 18 to 75 and above in the outpatient care setting.
What You'll Do
- →The Prevention Care Coordinator Registered Nurse is responsible for providing competent, evidence-based care to assigned patients.
- →Provides care coordination and planning for staff education and plan development for all prevention measures including: Pneumococcal vaccine, TD/Tdap Immunization, Zoster Immunization, Breast Cancer Screening and follow up, Cervical Cancer Screening and follow up, CRC Screening, Flu Immunization Age 66+, Flu Immunization ages 19-65, Tobacco Use Screen, Non-Rec PSA Screen.
- →The Care Coordinator assists PACT teams with programs and education to meet complex health care needs of their Veterans.
- →Coordinating and tracking breast care/mammography and cervical cancer screening of patients throughout all aspects of the Loma Linda Health Care System, including Community Based Outpatient Clinics.
- →The Care Coordinator assists Veterans, family members, and caregivers with receiving the most appropriate options and services to meet their complex health care needs.
- →Coordinates care with multiple providers across all levels and sites of care.
- →Coordination also includes assuring services interface within the Veterans Affairs (VA) and community programs to provide continuity of care to patients.
- →Assesses, plans, implements, and evaluates care based on age-specific components.
- →Assumes responsibility for the coordination of preventive care in primary care focused on patient education, self-management, and customer satisfaction throughout the continuum of care.
- →Follows procedures per established policies and guidelines.
- →Influences care outcomes by collaborating with members of the interdisciplinary team.
- →Core elements of performance include knowledge and active participation in the unit/program level quality improvement process and initiatives as well as customer service programs.
- →Analyzes and manages laboratory and radiology results and provides information to the providers.
- →Utilizes databases to provide population management to track patient care through all episodes of care.
Requirements
- ✓Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
- ✓All applicants tentatively selected for VA employment in a testing designated position are subject to urinalysis to screen for illegal drug use prior to appointment.
