About the Role
The Cincinnati VA Medical Center is recruiting for a Patient Aligned Care Team (PACT) Registered Nurse (RN) at its Georgetown, OH Community Based Outpatient Clinic (CBOC). The PACT RN executes position responsibilities that demonstrate leadership, experience, and creative approaches to management of complex primary care. The incumbent will provide sex-specific direct Veteran care and care coordination, as it pertains, to Veterans and their care within the Primary Care Department.
What You'll Do
- →The Cincinnati VA Medical Center (CVAMC) is a two-division campus located in Cincinnati, OH and Fort Thomas, KY serving 15 counties in Southwest OH, Northern Kentucky, and Southeast Indiana with 6 Community Based Outpatient Clinics located in Bellevue, KY; Florence, KY; Lawrenceburg, IN; Hamilton, OH; Clermont County, OH, and Georgetown, OH.
- →The CVAMC also supports two off main campus locations in the Cincinnati area via an Eye Center and Mental Health Outreach Community Division.
- →The major duties of the Registered Nurse (PACT) include, but are not limited to: Responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care.
- →Assesses, plans, implements, and evaluates care based on age specific components.
- →Assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care.
- →Administers medications and procedures per established policies and guidelines.
- →Influences care outcomes by collaborating with members of the interdisciplinary team.
- →Executes position responsibilities that demonstrate leadership, experience, and creative approaches to management of complex client care.
- →Manages assigned panel(s), utilizes registries, and ensures appropriate evaluation and access is provided to patient panels.
- →Provides health promotion, health education and coaching on wellness, disease prevention, and chronic care management.
- →Collaborates in the development, implementation, evaluation, and revision of policies, procedures, and/or guidelines.
- →Demonstrates performance and leadership that is broad enough to improve the care for the Primary Care Department and facility.
- →Provides peers with formal or informal constructive feedback for improvement.
- →Supports colleagues and other nurses through primary care knowledge sharing to provide safe, quality nursing care.
Requirements
- ✓Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
- ✓All applicants tentatively selected for VA employment in a testing designated position are subject to urinalysis to screen for illegal drug use prior to appointment.
- ✓Applicants who refuse to be tested will be denied employment with VA.
- ✓Selective Service Registration is required for males born after 12/31/1959.
- ✓Subject to background/security investigation.

- ✓Must pass pre-employment physical examination.
- ✓Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP).
