About the Role

The LTC Charles S. Kettles VA Medical Center is seeking a Full-Time Registered Nurse (RN) for a Otolaryngology (ENT) Clinic Care Coordinator position, aligned under Surgery service.

What You'll Do

  • The Surgery Clinic Care Coordinator is accountable directly to the Surgery Services Clinic Nurse Manager, Associate Chief Nurse for Surgery and Perioperative services, and ultimately to the ADPCS, functions in the surgery clinic and works to coordinate pre and post-operative care for surgical patients.
  • Duties include, but are not limited to: Responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care.
  • Assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care.
  • Administers medications and procedures per established policies and guidelines.
  • Influences care outcomes by collaborating with members of the interdisciplinary team.
  • Knowledge of the organization, the surgical process, and an in depth knowledge of the assigned surgical section's surgical procedure requirements.
  • Responsible for the development and maintenance of a system of performance measures to evaluate and enhance the overall effectiveness of the delivery of care in the Surgery Clinic.
  • Provides patient care, care coordination, health promotion, health education and coaching on wellness, disease prevention, and chronic care management.
  • Assures that appropriate documentation reflects patient assessments and interdisciplinary treatment plans in preparation for surgery or minor clinic procedures.
  • Demonstrates leadership in delivering and improving holistic care to adult and geriatric patients, through collaborative strategies, in compliance with the Michigan Nurse Practice Act, Medical Center, and VAHQ standards of practice.
  • Coordinates changes in the clinic schedule with the patient and clinic staff; ensure timely documentation; organize consults, coordinate equipment/RME needs to increase clinic utilization and throughput.
  • Provides education to surgical clinic patients as directed by the NPs and Chief of the service; coordinates multiple consult/appointment follow ups as needed, Makes routine patient return phone calls.
  • Identifies red flag phone calls or patient issues to bring to the attention of NP or resident/attending staff.
  • Manages clinic profiles, updates Service Agreements, consult completion, Encounter Action Required report, and daily workflow of the clinic.

Requirements

  • Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.

  • Applicants who refuse to be tested will be denied employment with VA.
  • Selective Service Registration is required for males born after 12/31/1959.

  • Must pass pre-employment physical examination.
  • Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP).
