About the Role
VA Greater Los Angeles Healthcare System (VAGLAHS) is seeking to hire a experienced Registered Nurse to work in our Homeless Patient Aligned Care Teams (HPACT). RN Care Coordinator is a member of the Teamlet and is responsible for collaborating with services internal/external to the VA to facilitate care transition in order to effectively meet the patient's needs, provides patient and family health education focusing on self-management, prevention, and wellness, based on the patient's goals.
What You'll Do
- →Current Greater Los Angeles VAMC employees will be given 1st consideration.
- →SF-50 must be submitted as proof of current employment.
- →If you do not submit SF-50, you will not be given 1st consideration.
- →Job duties include, but are not limited to, the following: Conduct biopsychosocial assessments to determine needs of Veterans and family members.
- →Participate in the treatment planning process with other disciplines.
- →Participate in discharge planning with other disciplines and will be responsible for ensuring that discharge plans are executed in a manner that is timely and appropriate.
- →Provide consultation/education to Veterans and families on community resources, advance directives, and VA benefits.
- →Act as a consultant to other team/staff members.
- →Act as liaison between VA and community resources, to include outreach duties as needed.
- →Provide case management to Veterans and families, as needed, throughout the continuum of care.
- →Provide a full range of services within commonly accepted standards of practice which includes case work, individual, family and group counseling, case management and other services as appropriate in community-based settings.
- →Assist with medication management as needed and appropriate.
- →Address Advance Directives in accordance with the standards of their work area.
- →Provide wellness/prevention education.
Requirements
- ✓Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
- ✓All applicants tentatively selected for VA employment in a testing designated position are subject to urinalysis to screen for illegal drug use prior to appointment.
- ✓Applicants who refuse to be tested will be denied employment with VA.
- ✓Selective Service Registration is required for males born after 12/31/1959.
- ✓Subject to background/security investigation.
- ✓Selected applicants will be required to complete an online onboarding process.
- ✓Acceptable form(s) of identification will be required to complete pre-employment requirements (https://www.uscis.gov/i-9-central/form-i-9-acceptable-documents).
- ✓Effective May 7, 2025, driver's licenses or state-issued identification cards that are not REAL ID compliant cannot be utilized as an acceptable form of identification for employment.
- ✓Must pass pre-employment physical examination.
- ✓Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP).
- ✓You may be required to serve a probationary period.
