About the Role
The Home Based Primary Care (HBPC) Outpatient Staff Registered Nurse (RN) is responsible and accountable for individual nursing practice and determines the appropriate delegation of tasks consistent with the nurse's obligation to provide optimal outpatient care. This nurse provides direct Veteran care and care coordination, as it pertains, to Veterans and their care within the HBPC Program.
What You'll Do
- →The HBPC RN assumes the following major duties and responsibilities: 1.
- →Develops and manages the individualized patient plan of care to include ongoing assessment, care planning, care coordination, monitoring, and evaluating clinical outcomes. 2.
- →Develops knowledge of Whole Health practices and provides health teaching, health counseling, provision of care that is restorative to life and well-being. 3.
- →Provides skilled primary care nursing interventions in the home such as vaccinations, urinary catheter changes/maintenance, venipuncture/specimen collection. 4.
- →Provides comprehensive medication management, education, and monitoring. 5.
- →Documents patient care assessments, interventions, care coordination, quarterly care plans, and patient outcomes in the electronic medical records according to established time frames. 6.
- →Completes nursing home safety assessments including need for adaptive equipment, environmental or behavioral concerns for staff or patient safety, and plans for emergency/disaster management. 7.
- →Plans, schedules, and completes nursing visits in the home, clinic, by phone, or using approved video technology according to established productivity measures and patient plan of care. 8.
- →Provides novice level nursing care management and coordination through communication with the HBPC Interdisciplinary team, specialty clinics/services, inpatient acute care, transportation services, community home care agencies, community providers/hospitals, caregivers, skilled nursing facilities, and RCFE staff. 9.
- →Preferred Experience: Three years recent full time experience as an RN One year nursing practice working with complex patient populations in the home care environment.
- →RN Certification in one of the following: Case Management, Geriatrics, Home Health Experience working in an autonomous nursing role.
- →Experience in home venipuncture and IV Therapy.
- →Contributions to performance improvement or evidenced-based activities in nursing practice.
- →Training in Shared Governance, Relationship Based Care, and/or High Reliability Organizations.
Requirements
- ✓Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
- ✓All applicants tentatively selected for VA employment in a testing designated position are subject to urinalysis to screen for illegal drug use prior to appointment.
- ✓Applicants who refuse to be tested will be denied employment with VA.
- ✓Selective Service Registration is required for males born after 12/31/1959.
- ✓Subject to background/security investigation.

- ✓Effective May 7, 2025, driver's licenses or state-issued identification cards that are not REAL ID compliant cannot be utilized as an acceptable form of identification for employment.
- ✓Must pass pre-employment physical examination.
- ✓Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP).
- ✓You may be required to serve a probationary period.
