About the Role
The Community Nursing Home (CNH) RN is administratively accountable to the Geriatric and Extended Care (GEC) Program Manager and clinically accountable to the GEC Nurse Manager for nursing specific issues. The incumbent is responsible for ensuring CNH Veterans care management needs are identified and plans are made to meet these needs through active collaboration with approved CNH facilities as well as other health professionals within the interdisciplinary team and in the community.

- →Assess and evaluate ongoing nursing home placement needs, making recommendations for discharge planning, coordination of care, and ensuring facility adherence to treatment plan.
- →Maintain standards of patient care and documentation guidelines established by national and local policy.
- →Provide nursing input for interdisciplinary health care plans to support Veteran-centric care.
- →Maintain current knowledge of medications and therapeutic procedures.
- →Review and assist in referrals to approved CNH facilities to ensure continuity of qualified comprehensive care.
- →Make care management visits to Veterans placed in approved CNH facilities on a regularly occurring basis, per the CNH Field Guidebook (FGB).
- →Recognize emergency situations and responds timely and appropriately.
- →Intercede on behalf of Veterans and families to facilitate optimum health care from approved CNH facilities and resources.
- →Participate in performance improvement activities.
- →Provide consultation to other nursing staff, other health care system services, and nursing home care personnel regarding Veteran needs in approved CNH facilities.
- →Communicate nursing care plans and Veteran care needs to family, home health agencies, and extended care facilities and VA clinics.

Requirements
- ✓Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
- ✓All applicants tentatively selected for VA employment in a testing designated position are subject to urinalysis to screen for illegal drug use prior to appointment.
- ✓Applicants who refuse to be tested will be denied employment with VA.

- ✓Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP).
