About the Role
The VA Greater Los Angeles Healthcare System (VAGLAHS) is seeking an experienced registered nurse to work in our Community Nursing Home (CNH) program. This program emphasizes on improving health outcomes through a system of case management and coordination of services for Veterans who are placed in VA Contracted Nursing Homes.
What You'll Do
- →Typical duties include but are not limited to: Serves as a liaison to Veterans, their families, Community Nursing Homes (CNH), VA Medical Centers, and any other Federal, State, or Private agencies as warranted.
- →Requires weekly driving.
- →Case manages a caseload of Veterans in the CNH program.
- →This includes monthly CNH visits with follow-up documentation in the Computerized Patient Record System, monitoring overall quality of care in the CNH's, Assessment for continued need and level of CNH care services, making referrals for specialty outpatient services, supplies and equipment and arranging transportation.
- →Participates in CNH program staff development, orientation processes and monthly meetings.
- →Reports Quality Improvement data, sentinel events or adverse patient occurrences, and participates in the development of policies and procedures.
- →Provide assistance in CNH discharge planning as well as coordinating placement within CNH when indicated.
- →Responds to outside referrals such as community hospitals, community nursing homes, veterans and family members living at home and any other agencies, such as Hospice, etc.
- →Develops and implements a plan to meet learning needs, instructing patients and families in basic health teaching and discharge planning.
- →Documentation is consistent, accurate, concise, and timely Revise's plan or care according to evaluations of patient's responses and outcomes Organizes and completes own assignments in a safe and timely manner, assisting team members as necessary.
- →Can recognize the patient has a problem Can manage the problem safely and effectively, recognizing his/her scope of practice.
- →Recognizes situations that require urgent response.
- →Demonstrates safe work practices.
- →Complies with safety rules and regulations for safe job performance and fire protection.
Requirements
- ✓Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
- ✓All applicants tentatively selected for VA employment in a testing designated position are subject to urinalysis to screen for illegal drug use prior to appointment.
- ✓Applicants who refuse to be tested will be denied employment with VA.
- ✓Selective Service Registration is required for males born after 12/31/1959.
- ✓You may be required to serve a probationary period.
- ✓Subject to background/security investigation.
- ✓Selected applicants will be required to complete an online onboarding process.
- ✓Must pass pre-employment physical examination.
- ✓Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP).
- ✓Participation in the Coronavirus Disease 2019 (COVID-19) vaccination program is a requirement for all Veterans Health Administration Health Care Personnel (HCP) - See "Additional Information" below for details.
- ✓Must be able to push, lift and pull up to 35 pounds.
