About the Role
The VA Texas Valley Coast Bend Health Care System is seeking a Registered Nurse Case -Caregiver Support Coordinator in the Caregiver Support Program (CSP) in Corpus Christi, TX.
What You'll Do
- →The Registered Nurse (GEC) will execute position responsibilities that demonstrate leadership, experience and creative approaches in providing complex patient management.
- →Responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care.
- →Assesses, plans, implements, and evaluates care based on Mental Health (MH) specific components.
- →Assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care.
- →Assesses, coaches, and supports the caregiver to perform the skills necessary to deal with the Veteran's mental illness needs.
- →Provides education and in-service training to VA Health System employees, Veterans, family, and Caregivers in the CSP.
- →Demonstrates advanced clinical knowledge in assessing planning, implementing, documenting, and evaluating care for a designated group of patients.
- →Initiates follow-up telephone calls appropriately and documents interactions on encounter form/electronic medical record.
- →Documents patient instructions and advice given.
- →Provides Care Management to patients with Ambulatory Care Sensitive Conditions (ACSCs) including reviewing the patient record, decides if the patient needs more in-depth education (formal or informal); nursing visits and/or telephone calls between provider visits to assist in care management.
- →Collaborates with the High-Risk Care Coordinators, Home Telehealth, and/or other referrals for care management.
- →Screens patients for social service, home care, and other community care needs; and coordinates or makes referrals as appropriate; and seeks consultation when indicated.
- →Completes and documents secure messaging, as applicable, in a timely manner.
- →Escalation of secure messaging does not exceed the VHA standard.
Requirements
- ✓Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
- ✓All applicants tentatively selected for VA employment in a testing designated position are subject to urinalysis to screen for illegal drug use prior to appointment.
- ✓Applicants who refuse to be tested will be denied employment with VA.
- ✓Selective Service Registration is required for males born after 12/31/1959.
- ✓Subject to background/security investigation.
- ✓Selected applicants will be required to complete an online onboarding process.
- ✓Acceptable form(s) of identification will be required to complete pre-employment requirements (https://www.uscis.gov/i-9-central/form-i-9-acceptable-documents).
- ✓Effective May 7, 2025, driver's licenses or state-issued identification cards that are not REAL ID compliant cannot be utilized as an acceptable form of identification for employment.
- ✓Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP).
