About the Role
This position falls under the Mental Health service line in the Outpatient Mental Health (OPMH) setting. Primary functions are case management and care coordination for Veterans who meet DSM-V diagnostic criteria. Veterans are likely to present with co-morbid medical conditions, functional decline, and unstable living conditions. The position is a part of an interdisciplinary team of mental health professionals referred to as a Behavioral Health Integration Program (BHIP).
What You'll Do
- →A Career At VA Salt Lake City Health Care System Can Take You Places (youtube.com) Duties and responsibilities include, but are not limited to the following: Provides psychosocial support, assists with problem solving and development of coping skills, and supports the Veteran's personal goals for recovery.
- →Obtains a health history of referred patients; data collection includes lifestyle patterns, pain issues, sleep/activity levels, support systems, cultural, social, and ethnic factors, patient treatment preferences and goals.
- →Administers long-acting injectable medications, including antipsychotics and opiate antagonists.
- →Administers screening tools, including PHQ9, GAD, PCL, MOCA, Suicide Risk Assessment, and Audit C.
- →Monitors ongoing treatments for adherence, medication side effects, and clinical outcomes, including worsening of symptoms, impairment, and therapeutic response, and provides and/or coordinates appropriate follow up.
- →Assesses patients and provides additional support between visits with licensed independent providers (LIPs).
- →Refers to, consults with and coordinates care with other VA health care providers.
- →Serves as liaison for Veterans in community hospitals and treatment settings to facilitate continuity of care.
- →Coordinates with other agencies and makes referrals to provide patients with necessary community services.
- →Functions as an advocate for Veteran self-determination.
- →Provides Veterans and their family members with education regarding their illness, medications, and treatment plan.
- →Facilitates psycho-educational and support groups.
- →Actively participates in team meetings and other MH meetings as appropriate.
- →Assists with education and acts as preceptor for others who provide nursing care, i.e. nursing students, new hires, and Peer Support Specialists.
Requirements
- ✓Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
- ✓All applicants tentatively selected for VA employment in a testing designated position are subject to urinalysis to screen for illegal drug use prior to appointment.

- ✓Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP).
