Provides direct patient care through comprehensive medication therapy management, prescribing within an approved scope, and collaborating closely with primary care teams to optimize therapeutic outcomes. Applies evidence-based clinical judgment to manage complex drug regimens, ensure formulary compliance, and enhance safety and efficacy. Uses telehealth tools when needed, provides education to patients and staff, and supports continuity of care to improve overall health outcomes.

- →Incumbent will initially be reporting to the Northwest Health Care Center with expectation to transition in-person to New Braunfels clinic if/when space becomes available.

Requirements
