About the Role
This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application. Former EDRP participants ineligible.
What You'll Do
- →The Clinical Pharmacy Specialist (CPS) -Primary Care PACT functions as a member of their assigned PACT Teams.
- →The CPS works independently under their individual scope of practice to provide direct patient care.
- →The CPS primarily works at the James J.
- →Peters VA Medical Center but may be asked to work at any of the community based outpatient clinic sites if needed.
- →Duties include and are not limited to: Serves as an independent practitioner in the ambulatory setting requiring the highest level of pharmacist clinical privileges with minimal supervision.
- →Performs administrative, direct patient care, and non-direct patient care activities.
- →Utilizes different modalities to provide care as per facility guidelines including telehealth appointments.
- →Participates in medication utilization evaluations and other quality assurance activities to ensure appropriate and safe medication use.
- →Directly assists other providers in rational medication selection and monitoring by providing written and/or verbal consultation.
- →Participates in formal and informal educational programs of the service, including serving as a role model and preceptor to pharmacy students and residents.
- →Work Schedule: Monday - Friday (8:00am - 4:30pm), with rotating evenings, weekends, and holidays Telework: Not Available Virtual: This is not a virtual position.

Requirements

- ✓Must pass pre-employment physical examination.
- ✓Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP).
