About the Role
This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application. Former EDRP participants ineligible to apply.
What You'll Do
- →VA Palo Alto Health Care System (VAPAHCS) has an immediate opening for a Patient Aligned Care Teams (PACT) Ambulatory Care Clinical Pharmacy Practitioner (CPP) in Palo Alto.
- →This pharmacist performs clinical functions independently and maintains a scope of practice in specific therapeutic disease states (including, but not limited to: Diabetes, Hypertension, Hyperlipidemia, Congestive Heart Failure, Chronic Obstructive Pulmonary Disease, Anticoagulation, etc.).
- →This pharmacist is an integral member of the PACT and works collaboratively with other CPPs and the interdisciplinary team, including medical residents and fellows.
- →SAFE AND EFFECTIVE MEDICATION USE: Enforcing VHA treatment and formulary guidelines; providing education to providers to promote appropriate prescribing patterns and adherence to VHA formulary; documenting clinical work; and meeting HEDIS, SAIL, and other performance metrics.
- →OTHER: Precepting PGY1 residents and pharmacy students as assigned; and serving on local and VISN committees as assigned.

Requirements
