What You'll Do

  • The CPS - Ambulatory Care practices in the primary care setting.
  • Additional CPS functions include but are not limited to performing formulary utilization and quality assurance reviews, addressing drug information questions and other specified clinical functions.
  • The CPS may be assigned to other pharmacy related services to include the outpatient and inpatient pharmacy when necessary.
  • The CPS is a staff member of the Pharmacy Clinical Service Line, supporting the department's mission, goals, policies and procedures and communicates directly with Pharmacy Clinical Service Line directors, supervisors and managers.
  • Functions Provides direct patient care in an ambulatory care setting to manage medication therapy and disease outcomes, functioning autonomously and collaboratively.
  • Interviews Veterans to obtain and document comprehensive medical and medication histories.
  • Conducts basic physical assessments and orders necessary laboratory tests to evaluate safety and efficacy of medication therapy.
  • Designs, implements, modifies, and documents individualized pharmaceutical care plans based on clinical assessments Initiates, adjusts, and discontinues medications to accomplish patient-specific therapeutic goals.
  • Performs comprehensive medication reconciliation to enhance safety, reduce polypharmacy, and provide updated medication lists; educates Veterans on medication changes.
  • Communicates care plans, significant findings, and medication updates to Veterans, caregivers, and health care team members to ensure continuity of care.
  • Serves as a liaison between Pharmacy Services and other healthcare providers.
  • Reviews and adjudicates non-formulary drug requests, participates in formulary management, and implements formulary changes, drug safety alerts, and conversion plans.
  • Participates in Adverse Drug Reaction (ADR) and medication error reporting initiatives to promote Veteran safety.
  • Provides expert drug information, dosing recommendations, and clinical guidance to health professionals and pharmacy staff, including pharmacokinetic and renal/hepatic dose adjustments.
