About the Role
The Visual Impairment Service Team (VIST) Coordinator manages patient care and provides care coordination, assessments, and blind and low vision rehabilitation to help Veterans reclaim the confidence and skills needed to lead an independent, fulfilling life after vision loss.
What You'll Do
- →The position may be based in Marion, IL or Evansville, IN The VIST Coordinator plans, develops, implements, directs, and evaluates the Visual Impairment Service (VIS) Team program at the VHA sites within his/her catchment area.
- →Case management duties may include cases that are both difficult and complex both in terms of the severity of the visual impairment and the presence of complicating physical and emotional factors impinging on treatment.
- →The Visual Impairment Service Team (VIST) Coordinator functions independently in a number of settings with limited supervision at the main facility, EHCC and all Community Based Outpatient Centers that fall under the catchment of the Marion VAMC.
- →The incumbent MUST maintain a valid state issued driver's license.
- →The position may require minimal travel into the community to meet with community agencies and partners who assist and support the VIS Team, its Veterans and family members.
- →Occasionally, the VIST Coordinator may travel for home visits.

Requirements

- ✓Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP).
