About the Role

This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application. Former EDRP participants ineligible to apply.

What You'll Do

  • VA Careers - Social Work: https://youtube.com/embed/enRhz_ua_UU Collaboration with inpatient treatment providers to support appropriate discharge planning following recommendations for treatment of patients at high risk for suicide.
  • Assuring that all Veterans assigned to the facility high risk monitoring list are being followed for a time period not less than 90 days following hospitalization, or 90 days from the date of assignment to the high-risk list.
  • Monitoring activities may include: Face to face and/or telephone contact with the Veteran to assess for safety and ongoing compliance with treatment.
  • Tracking any no-show appointments by these patients and following up with the patient and provider as soon as possible to assess safety and reschedule the appointment.
  • If the safety of a high risk Veteran is unable to be established, or it is determined that the Veteran is at imminent risk for self-harm, initiating emergency interventions appropriate to the situation, including but not limited to, contacting Veteran's identified emergency contact, and /or requesting a safety and welfare check.
  • With patient's consent, contacting family members and/or other identified supports to provide education and involve in patient's treatment and safety planning.
  • Contacting and collaborating with the Veteran's primary mental health providers to coordinate monitoring activities, track appointments, and coordinate enhanced care as needed.
  • Making recommendations for referral of repeat attempters to a variety of appropriate programs such as Peer Support, RANGE, PRRC, and specialty mental health care.
  • Maintaining awareness of suicide treatment options and making recommendations to the patient's provider when new options become available.
  • Assuring that all high-risk Veterans have completed and received a copy of a Suicide Prevention Safety Plan which documents crisis prevention strategies that the Veteran identifies and agrees with.
  • When it is clinically determined that a Veteran is no longer at high risk, the case manager collaborates with the primary therapist to oversee the transition of care (if needed) to assure that appropriate behavioral health follow up is in place.
  • Collecting and reporting data related to suicide behaviors, safety plan completion, outreach activities, and high-risk monitoring as required by local administration, VISN, and VACO.
  • Responding to consults from the Veterans Crisis Line.
  • Assisting with provision of on-going Guide Training program (Operation SAVE) for new employees and non-clinical frontline staff such as clerks and telephone operators as needed to assure that they are aware of options for patients with whom they come in contact during crisis situations.
