About the Role

The incumbent serves as the Homeless Care Line Programs Manager for the VA NY /NJ Healthcare Network (VISN. The incumbent reviews, plans and coordinates the delivery of homeless programs across this network of 9 facilities (health care systems) to achieve VISN 2's mission. As the Care Line Programs Manager, the incumbent is actively involved in all phases of homeless program development, implementation, evaluation, strategic planning and coordination.

What You'll Do

  • This individual effectively contributes to the VA and the Network's strategic objectives by developing, implementing, coordinating and providing administrative and clinical oversight to programs.
  • The individual works with and through homeless leaders at all VISN 2 sites as well as other governmental and/or community-based agencies and organizations that serve homeless Veterans.
  • The incumbent is responsible for representing the VISN 2 Homeless Care Line and is expected to function independently.
  • The network provides a wide range of services to include, but not limited to primary, specialty and mental health care, homeless outreach and support, telehealth technologies and services in partnership with community care providers.
  • The network works in partnership with a myriad of academic affiliations to include physician, nursing, pharmacy, social work, health care administration, and physical therapy.
  • The incumbent serves as the Homeless Care Line Programs Manager for the VA NY/NJ Healthcare Network (VISN 2).
  • The incumbent reviews, plans and coordinates the delivery of homeless programs across this network of 9 facilities (health care systems) to achieve VISN 2's mission.
  • As the Care Line Programs Manager, the incumbent is actively involved in all phases of homeless program development, implementation, evaluation, strategic planning and coordination.
  • The VISN 2 Homeless Care Line Manager administers information and analytical systems to evaluate and enhance the quality of care provided to patients.
  • Develops and implements information systems to track care activities including visits, referrals, patient demographics, program utilization, resource capacity, and patient satisfaction.
  • The VISN 2 Homeless Care Line Manager leads in the development of network-wide guidelines for program standardization of data capture and service delivery for Homeless Programs.
  • Incumbent is responsible for developing mechanisms for monitoring compliance with associated performance measures as well as developing a system for standardized outcome assessment across programs.
  • Develops, implements, and evaluates clinical guidelines and protocols to establish appropriate utilization of Homeless Care Line programs.
