Social Worker (Intensive Community Mental Health Recovery Case Manager)

About the Role
The incumbent works for the Central Virginia VA Health Care System, Health Care Center (FxHCC) in Fredericksburg, Virginia, as a direct report to the Social Work Service and the Lead Social Work Supervisor, and is assigned to the Intensive Community Mental Health Recovery (ICMHR) Program, also known as Mental Health Intensive Case Management ( MHICM ) program.
What You'll Do
- →Provides community-based, intensive case management and therapeutic services to Veterans diagnosed with serious mental illness, especially individuals dealing with or previously diagnosed with substance use disorder or other substance related dependency.
- →Intensive case management services are characterized by assertive Outreach, individualized treatment planning and multidimensional orientation and intensive delivery of services in the Veteran's home or community.
- →In coordination with the ICMHR interdisciplinary team, manages a case load of clinically and psychosocially complex Veterans admitted to the ICMH program.
- →Has training and experience working with individuals dealing with substance use disorders in special populations and with co-occurring serious mental illness.
- →Will work closely with the ICMHR Program Coordinator, the SATP Program Coordinator, the assigned Mental Health Prescriber, Psychiatry, and the SATP Psychologist to collaboratively determine the best treatment plan and supportive services to meet the ICMHR program Veteran's needs.
- →Will also work closely with the PRRC program staff to find supportive solutions to meet the complexities of the ICMHR Veteran panel, by providing rehabilitative services through the VA and with community partners.
- →Performs other directly related duties as assigned.
