About the Role

The Montana VA Health Care System, Mental Health Service is currently recruiting for one (1) Senior Social Worker - Intensive Community Mental Health Recovery Program (ICMHR) Case Manager, at the Fort Harrison VA Medical Center, in Fort Harrison, Montana.

What You'll Do

  • Total Rewards of a Allied Health Professional The Senior Social Worker - ICMHR Case Manager delivers intensive, community-based case management to Veterans with serious mental illness and high inpatient use, supporting recovery, independence, and community integration.
  • Working autonomously, duties include assessment, treatment planning, therapy, placement, and crisis intervention, with interventions tailored to each Veteran's needs and goals.
  • Services are recovery-oriented and primarily provided in the community.
  • Major duties and responsibilities of the position include but are not limited to: Provide individual, group, and family counseling to patients and families experiencing complex emotional, medical, and social issues.
  • Independently assess psychosocial functioning and needs of patients and families.
  • Formulate and implement individualized treatment plans, identifying problems, strengths, coping skills, and support needs.
  • Utilize supportive counseling, problem-solving, and crisis intervention techniques to manage and support mental health crises.
  • Complete screenings and assessments of Veteran referrals, determine clinical eligibility, and present findings to the ICMHR treatment team.
  • Deliver intensive case management in Veterans' homes and communities, emphasizing rehabilitation, crisis/stress management, skill building, and increased independent functioning.
  • Provide advanced case management and specialized treatment for Veterans with serious mental illness and co-occurring conditions.
  • Collaborate with VA and community agencies/stakeholders to ensure continuity of care and act as a resource and liaison.
  • Address and resolve Veteran needs or concerns through effective collaboration.
  • Operate government vehicles to conduct home/community visits or transport Veterans as part of care plans.
  • Work Schedule: Monday - Friday, 8 AM - 4:30 PM Compressed/Flexible: May be available at supervisory discretion.

Requirements
