About the Role
The Women's Health Clinical Navigator focuses on individual growth and development in practice with increased levels of self-direction. This nurse brings principles and theory with limited application of critical thinking skills. Provides patient management, care coordination, and disposition planning for Veterans, including but not limited to addressing mammography, cervical cancer screening, and/or bone density testing.
What You'll Do
- →The Women's Health (WH) Clinical Navigator will interface with outside referral agencies and ensure that their business and clinical practices follow regulatory standards.
- →Anticipates and implements preventive patient care measures.
- →Provides guidance and education to patients and caregivers and collaborates with other professionals at all levels.
- →Supports the WH Program and evaluates care and outcomes to ensure timely and appropriate provision of gender specific services.
- →Advocates for holistic care through patient/family education and provider interaction and implements an educational plan to meet the patient care needs.
- →Provides patient management, care coordination, and disposition planning for Veterans, including but not limited to: Coordinates care with multiple providers across all levels and sites of care.
- →Addresses mammography, cervical cancer screening, bone density testing and treatment, maternity care coordination and reproductive health.
- →Provides oversight of the patient's care through the diagnostic and treatment continuum, ensuring that all steps in the process are scheduled timely and completed.
- →Monitors the provision of services and tracking of outcomes.
- →Retrieves statistical data on care and utilization at the local VAMC using Veterans Health Administration (VHA) databases.
- →Anticipates and implements preventive patient care measures.
- →Provides peers with informal constructive feedback for improvement.
- →Fosters a safe and supportive environment conducive to the professional development of healthcare professionals.
- →Subject to change based on needs of the facility.
Requirements
- ✓Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
- ✓All applicants tentatively selected for VA employment in a testing designated position are subject to urinalysis to screen for illegal drug use prior to appointment.
- ✓Applicants who refuse to be tested will be denied employment with VA.
- ✓Selective Service Registration is required for males born after 12/31/1959.
- ✓Subject to background/security investigation.
- ✓Selected applicants will be required to complete an online onboarding process.
- ✓Acceptable form(s) of identification will be required to complete pre-employment requirements (https://www.uscis.gov/i-9-central/form-i-9-acceptable-documents).
- ✓Effective May 7, 2025, driver's licenses or state-issued identification cards that are not REAL ID compliant cannot be utilized as an acceptable form of identification for employment.
- ✓Must pass pre-employment physical examination.
- ✓Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP).
- ✓You may be required to serve a probationary period.
